Anesthesiology: From Patient Safety to Population Outcomes

Kevin K. Tremper, PhD, MD

This talk will cover the "points of inflection" in the development of Anesthesiology, starting with the first public demonstration of ether anesthesia and the first mortality. The discussion will progress through the early development of documentation of vital signs and how that progressed to the development and implementation of monitoring systems in the 1980s and more recently, the development and implementation of perioperative anesthesia information systems (AIMS). This discussion will be compared to the growth of the aviation industry. The safety record of aviation and the efforts that have been made to progressively improve its safety will be compared with those in Anesthesiology. The monitoring standards presented in 1986 in the United States significantly improved anesthesia safety just as Federal Aviation Administration standards improved the safety of flight. Newer aircraft have developed fly-by-wire avionics, which enable the complex system of a modern jet to be monitored and for the pilot to maneuver under a variety of adverse conditions. Anesthesia information systems and the large national databases that will be developed will enable Anesthesiology and Medicine in general, to have similar "avionics for medicine," which we refer to as decision support. Because of the way anesthesiologists approach patients, completing a detailed review of systems as part of their risk assessment, we are the ideal physician group to develop these risk stratifying tools for care not only in the operating rooms but in all acute care settings.

Disaster and Mass Casualty Medicine is increasingly at the forefront of attention with numerous recent natural and man-made events.

The purpose of this workshop is to prepare health care providers to provide a more effective initial response to a mass casualty disaster, with lessons reinforced by the Haiti earthquake experience. Particular attention will be paid to three phases of Disaster and Mass Casualty: triage, treatment, and transportation.

The workshop will be organized into large group discussions and presentations with facilitated breakout sessions to work in small groups on solutions to complex scenarios. Small groups discussions will be facilitated by faculty with Disaster and Mass Casualty expertise.

As a result of this training, the participants should be able to: define mass casualty disasters, describe categories of mass casualty disasters, describe characteristics of the response, explain response to the disaster and identify planning and preparedness activities that will strengthen the response.

Acquire the indispensable skills to utilize each device for a specific use in the management of a difficult airway

Unit I - Flexible And Rigid Fiberoptic Techniches
Develop basic skills in basic fibrobronchoscopy for oral intubation and advanced management for nasal intubation. To develop skills on intubation with optic stylets.

Unit II - Extraglottic Devices
To develop dexterity in securing the airway without the need of intubation. To master the skills in the advanced use of extraglottic devices. To identify the indication for using an extraglottic device as a rescue technique. To describe extraglottic devices for intubation and its indications.

Unit III - Trans-Glottic Devices And Safe Extubation
To master skills in the use of intubating styles such as Frova and Bougie. To describe and complete a safe extubation technique. To use correctly a tube exchanger.

Unit IV - Video-Laryngoscopes
To acquire the skills to intubate using a channelled and non-channeled video-laryngoscope.

Unit V - Trans-Cutaneous Techniques
To successfully rescue a NINV situation. To accomplish in less than 60 seconds a cricothyrotomy on a model or an animal specimen. To master a trans-tracheal jet ventilation. To learn to perform a percutaneous tracheostomy.

After attending this workshop attendees will understand the important perioperative issues involving the patient with obesity and sleep apnea, and the new concepts in management of oxygenation during one-lung ventilation, the pulmonary HPV, and the pulmonary microcirculation. In addition, the recent advances in lung separation and post thoracotomy pain management will be discussed.

A) Introduction

In addition an adult and a paediatric simulators will be available for management of crises during thoracic anaesthesia.

Arterial and venous thrombosis are important causes of perioperative cardiovascular morbidity and mortality. In recent years many new agents have been developed with the vision to provide "better" anticoagulation and platelet inhibition.

Direct thrombin inhibitors (DTI), such as hirudin, bivalirudin, argatroban and dabigatran, Factor Xa inhibitors (xabans) such as rivaroxaban, and synthetic pentasaccharide such as fondaparinux are new anticoagulants. In particular, dabigatran (Pradaxa®) and rivaroxaban (Xarelto®) are the two most promising orally active agents which both seem to carry the promise of high efficacy, predictable pharmacokinetics, low requirement for coagulation monitoring and reasonably short duration of action upon drug discontinuation.

For antiplatelets, newer P2Y12 receptor inhibitors have been introduced recently. These include prasugrel (Effient®), and the most promising reversible P2Y12 antagonists cangrelor and ticagrelor. The platelet surface glycoprotein IIb/IIIa is another favourite target for platelet inhibition. Abciximab (Reopro®) and tirofiban (Aggrastat®) are two commonly used Gp IIb/IIIa antagonists while eptifibatide (Integrilin®) is the newest of this class and holds promise as a perioperative bridging anti-platelet agent due to its more flexible pharmacokinetics. Terutroban, a thromboxane (TP) receptor antagonist and the protease-activating recptor-1 (PAR-1) antagnosits such as atopaxar (E5555) are antiplatelets under Phase II/III trials and are worth keeping an eye on.

Overall, new anticoagulants and antiplatelets present both challenges and opportunities in the perioperative period. Developing proper monitoring of the effects of these drugs and choosing an agent most suitable for perioperative use will be of top priority.

SPANISH ANESTHESIA INCIDENT REPORTING SYSTEM

Reporting and learning from incidents is considered a key tool to improve patient safety. This lecture describes the introduction in Spain of a national anesthesia incident reporting system.

The system was born in January 2009 as SENSAR (Sistema Español de Notificación en Seguridad en Anestesia y Reanimación) with the support of the Spanish Society of Anesthesiology (SEDAR). It is a speciality-specific departmental system designed and used by anesthesiologists to improve the safety of patients under anesthesia care. It works as a multicenter, web-based, electronic system in which every hospital shares their incidents reports, analysis forms and learning and improving-measures adopted.

Its main characteristics are:

1. It is a anonymous and voluntary reporting system. Any event that produces or can produce an unnecessary harm to the patient can be reported. The system encourages the reporter to provide as much contextual information as possible.
2. The analysis is based in a systematic approach looking at the whole system and performed through a standarized framework and methodology.
3. Analysis and measures are implemented in every hospital. The analysers of each hospital can check the global database to learn from similar incidents reported by other hospitals.
4. The reporters can track any time the state and analysis of incidents reported by themselves.
5. There is feedback to the reporters as safety alerts, bulletins, reports, specific guidelines, clinical lectures, etc.

Nowadays more than 70 hospitals use the system, 3000+ incidents have been analyzed and 2200+ improving measures have been implemented.

Statins are known to reduce the risk of major coronary events, stroke, all-cause mortality, and the need for repeat revascularisation after PCI. They improve outcome in heart failure and appear to protect patients during critical illnesses. They are also effective in preventing perioperative complications of non-cardiac surgery1 and cardiac surgery2, including atrial fibrillation. Most of the evidence comes from observational studies in patients on chronic statin medication. Indeed, the largest study with over 77,000 out of 780,000 patients taking statins, showed a 30% reduction in all-cause mortality in those treated with statins3. Few randomised controlled trials have examined the deliberate introduction of statins prior to surgery4, 5: benefits were observed mostly in high risk patients. While the results are encouraging, there is need for more randomised controlled trials, but the widespread use of statins may make almost impossible.

Guidelines from the American College of Cardiology Foundation/American Heart Association (ACCF/AHA)6 and the European Society of Cardiology endorsed by the European Society of Anaesthesiology (ESC/ESA) 7 both recommend to continue therapy in patients currently taking statins. The ACCF/AHA guideline considers their introduction to be reasonable in patients undergoing vascular surgery and suggests that statins may be considered in patients with at least one clinical cardiac risk factor who are undergoing intermediate-risk procedures. By contrast the ESC/ESA guideline advocates to start statin treatment only in high-risk surgery patients, optimally between 30 days and at least one week before surgery.

The evidence upon which these recommendations are based, and their strength, will be discussed, including the role of the pleiotropic effects of statins in perioperative medicine.

Cardiovascular disease remains a major health care problem in the world. A high incidence of myocardial ischemia and infarction, encountered during the intra- or post-operative periods incited research efforts to develop protective interventions for decreasing cardiac risk in patients with cardiovascular disease. Parallel to the most powerful endogenous cardioprotective mechanism of ischemic preconditioning, pharmacologic preconditioning with volatile anesthetics emerged as a considerably less risk bearing and equally effective cardioprotective intervention. It is well documented that the detrimental impact of the ischemia/reperfusion episode can be substantially mitigated by pretreating the heart with short periods of volatile anesthetic administration. These preconditioning strategies are aimed at preserving mitochondrial bioenergetics during the metabolic stress, which leads to improved mitochondrial and cellular function with reduced necrotic and apoptotic cell death. The observation that volatile anesthetics protect ischemic myocardium is of important clinical significance for patients with coronary artery disease. Morbidity and mortality, secondary to perioperative myocardial ischemia, is disproportionately high. Current data indicate that anesthetic-induced preconditioning occurs and myocardium can be rendered more resistant to ischemia and reperfusion injury. Anesthetic-induced preconditioning displays many similarities to ischemic-induced preconditioning including a window of protection that has also been confirmed in patients undergoing coronary bypass graft and other surgeries. Indeed, the experimental evidence of anesthetic cardioprotection appears to translate to a clinically relevant cardioprotection with a beneficial effect on patients' outcome. Nevertheless, despite the promising data from the initial clinical studies, additional efforts are necessary to firmly establish these beneficial effects.

Assessing the severity of background diseases among patients with hip fractures may be inadequate by the difficulty in generating reliable communication with an elderly patient, the urgency of the operation and the absence of accurate medical history. Furthermore, the severity of the underlying heart disease is often unclear and diagnosis can be difficult because some patients remain asymptomatic, and even patients with left ventricular diastolic dysfunction may have normal cardiac size and left ventricular ejection fraction. Early recognition of preoperative ischemia or congestive heart failure (CHF) is frequently skewed by a parallel administration of analgesics or dyspnea of pulmonary origin.

Our hypothesis is that intensive perioperative management of patients with heart failure undergoing hip fracture repair will improve surgical outcome and reduce mortality. Our aims as anesthesiologists in CHF patient management in the perioperative setting are: a. early identification of patients in active CHF state and b. providing best preemptive medical care. The diagnostic task will be performed by reviewing patient's history and his current medications, by echocardiographic examination or dobutamine stress echocardiography, and by quantitative identification of the severity of the patient's heart disease using a relatively new bed-side blood test for NT-proBNP.

The Renin-Angiotensin-Aldosterone System (RAAS) is an important mediator in pathogenesis of CHF. Hypertension, diabetes and hyperlipidemia-induced release of angiotensin II through the stimulation of AT1 receptor will result in deleterious effects throughout the cardiovascular continuum, with end effect of remodelling of the myocardium and the development of heart failure. Thus medications such as angiotensin converting enzyme inhibitors, and diuretics such as spironolactone are an important therapy in CHF patients.

Important objective involves improving the patient's heart function during surgery, using multi-modal approach of monitoring and drug therapy targeted at augmenting myocardial performance. Such approach may reduce surgical risk and prevent postoperative cardiac, cerebral and renal complications, and may improve patient outcome in the first postoperative year.

A RATIONAL APPROACH TO PERIOPERATIVE FLUID MANAGEMENT

Applying crystalloids generously and avoiding colloids are main characteristics of traditional perioperative fluid handling. Recent data indicated that this approach leaving blood volume unchanged while increasing body weight significantly might be associated with perioperative complications [1]. From a pathophysiological standpoint this is not surprising. As, in contrast to the common assumption, fasting deficits [2] and insensible perspiration are negligible [3] and the existence of a relevant "third space" has never been proven [4], crystalloids, applied in traditional amounts to replace extracellular losses, must accumulate interstitially. Beyond that, basic physiology and direct measurements in humans revealed that these preparations load the interstitial space by 80% even when applied to stabilize cardiac preload in a bleeding patient. But also when using colloids for this indication one must carefully envision that hypervolemia deteriorates the endothelial glycocalyx, a key structure of the vascular barrier [5]. This leads to a continuing pathological protein-rich shift across the vascular wall, resulting in interstitial edema compromising microcirculation and oxygen delivery. As it most probably takes several days to restore the glycocalyx this problem should also influence postoperative intensive care and convalescence.

Liberal and undifferentiated fluid handling appears not to be a necessary therapy of unavoidable perioperative losses, but the primary cause of an avoidable shift towards the interstitial space. Using the right kind of fluid in appropriate amounts at the right time might improve patient outcome [1]. As outcome-based evidence is still incomplete, a practical approach to perioperative fluid handling in 2012 is physiology-based and evidence-supported [6].

Aims

  1. To consolidate the anatomy and physiology of paediatric airway compared with the adult.
  2. To review the pharmacology, sedation and anaesthetic techniques related to the paediatric airway management
  3. To recognize the anomalies and syndromes associated with compromise of the paediatric airway.
  4. To familiarize with current paediatric airway management devices (special attention to videolaryngoscopes)
  5. To practice different paediatric airway management approaches and techniques.
  6. To acquire the skills in the management of the paediatric broncho-fiberscope

Organization
The paediatric airway workshop will consist of four practical stations:
Station 1 - Supraglottic devices in paediatrics
Station 2- Paediatrics Videolaryngoscopes
Station 3 - Optical Stylettes
Station 4 - Paediatric flexible bronchoscope
